Vet Clinic Registration Checklist
The following checklist is for customers to ensure proper registration of their pets with the RAF Feltwell Vet Clinic. Services are only provided for customers that have completed the checklist.  Registration packets can be picked up at the Family Support Center or you can click on the link below to retrieve the equivalent DD2343 and follow the steps listed.

One copy of DD Form 2343 is required per pet. Please include a PSC address, physical address, and email address if available.  Please update your unit/squadron, installation, telephone numbers, and DEROS as soon as this information changes.

  1. One copy of orders assigning the individual to the UK (front only is fine)
  2. Please initial the first page and sign the second page of the policy letter
  3. Medical history only if pet had special requirements. (e.g. diabetes,
      hyperthyroidism, etc.)

Registration packets can be hand-delivered to the Feltwell Vet Clinic, faxed, e-mailed, or mailed. Packets sent through the mail should be sent to the following address:

General Rules and Regulations
We discourage bringing children under the age of 12 into the VET clinic for safety reasons. 
Customers should be on time. Clients that are 15 minutes late may be rescheduled. 
All animals must be on a leash or in a carrier. 
All clients are required to register their pets BEFORE they can make an appointment.  Registrations may take up to one week to complete after the correct information is provided.   

Services Offered

The RAF Lakenheath Veterinary Treatment Facility offers deworming, immunizations, micro-chipping, health certificates, nail-trimming and other services (such as sick call and surgical procedures) as time and mission permit. If you would like an appointment, please contact the RAF Feltwell Clinic in advance. To order flea and tick medications, de-worming, or heartworm prevention please have an accurate weight of your pet available when you call.  Special and prescription foods can be pre-ordered, paid in advance and picked up at any clinic locations.  Please specify where you would like to pick up your order when you call in the request.

Please inform us if you have any changes (telephone number, address, rank, pet ownership status etc) so we can maintain up-to-date records.  

Preventive Medicine/Vaccination Recommendation 

Vaccinations are a means of stimulating your animal's immune system to provide disease-fighting antibodies. A series of vaccinations given every 3 to 4 weeks should be administered to young dogs and cats from approximately 6-weeks to 6-months old.  Protection is maintained by "booster" vaccinations given either bi-annually, annually or every 3 years (different vaccinations require boosters at different intervals).  Vaccinations have saved the lives of many animals, and they also help to prevent zoonotic diseases.  It is never too late to start an animal on a vaccination schedule!

Revaccinations should be scheduled at 6, 12 or 36 months, depending on the vaccine.

Dogs should be vaccinated against canine distemper, infectious canine hepatitis, canine parvovirus, canine parainfluenza, and canine leptospirosis.

Cats should be vaccinated against feline panleukopenia, feline viral rhinotracheitis, and feline calicivirus. Vaccination for Feline leukemia is highly recommended for outdoor cats.

Although the UK is a rabies-free country, we recommend maintaining your animal’s rabies vaccination schedule.  This may help you avoid having to repeat the FAVN testing that was required to get your pet into the UK. 

We recommend you deworm your dog/cat regularly, using either the monthly heartworm preventatives (that also help control intestinal parasites) or with a broad-spectrum dewormer.

Rabbits in the United Kingdom are vaccinated annually against Myxomatosis and semi-annually to prevent viral hemorrhagic disease.  

Pet Passports 

Pet passports are not required to enter the United Kingdom from the United States nor are they required to go back. To enter the United Kingdom from the United States you need to follow the Pet Travel Scheme. For more information about this program, you should go to www.defra.gov.uk.  

You will need a pet passport if you are planning on traveling from the island of England/Scotland to Ireland or mainland Europe. The same steps as for the Pet Travel Scheme need to be followed to qualify for a pet passport. We can issue pet passports here at the RAF Feltwell Vet Clinic. Please print and fill out the Pet Passport application. Please make sure we have all the documents that go with the application when you turn it in. You can hand deliver it, mail (MPS) or email it to us. 

Travel Abroad 

If you know you are going to be stationed remotely (e.g. Hawaii, Japan, New Zealand) you will need to contact the DEFRA office in Carlisle (phone 01228-591999, fax 01228-591900).  DEFRA will provide you with a request for export forms to be filled out and returned to the DEFRA office.  DEFRA will then forward the export certificate paperwork to your vet.  

Bringing Your Pet to the UK

If you are PCSing to the UK, please see the DEFRA website (www.defra.gov.uk) or call the DEFRA office (0-11-44-1228-591999) to ensure you meet the Pet Scheme requirements.  Please also see the link "Shipping your pets to the UK."

Power of Attorney

If you need to have someone other than you or your spouse bring your animal into the Vet Clinic, you must have a special power of attorney filled out, signed and notarized.

Re-homing Your Pet

If you re-home your pet, please provide us with a signed statement of release to the new owner including the new owner’s name and contact information as well as the animal’s identification information (name, microchip number etc). Also, you must deregister the animal and the new owner must re-register the animal in their name.

Animal Welfare Officer

Yvonne Chadwick is the Animal Welfare Officer for RAF Lakenheath, RAF Feltwell. Please contact her if you find a stray animal, suspect animal abuse/neglect (DSN 314-226-5796, email Yvonne.chadwick.gbr@lakenheath.af.mil) or you have other animal welfare related concerns/questions.  On RAF Mildenhall, please contact Security Forces or the RSPCA (0300-1234-999) for suspected animal abuse/neglect cases or stray animal issues.
